Feedback Form

Use the feedback form to raise any questions, concerns, or suggestions to the TIPSY Board. You can remain anonymous if you wish. All feedback will be reviewed at board meetings.

Harassment Form

Use the harassment form to report and concerns about or violations of the Safer Space Rules. You can remain anonymous & the matter will be dealt with confidentially. Please also consider reporting any incidents to TREY.

Expense Claim Form

Use this form if you made a purchase on behalf of TIPSY and need to be reimbursed.

Please keep all receipts!

Vote Transfer Form

If you are unable to make a meeting where a vote takes place, you may use this form to grant someone else permission to vote on your behalf.

Please note that this form must be signed by both parties and sent 24 hours in advance if submitted by email.
